Extract to mods folder, then simply in yor cod4 player click setup and type your mod like
city_before_hero
Interchangable deagles, choose one from pistol_iwds, and replace z_normal_deagle.

Best works with my cfg also available for download.
City Before Hero is designed for Strike and works only on strike
Differs from "Strike Predators" with defence models, and gloves
